By checking this box, I authorize Primeak Inc. to perform a personal credit inquiry on the guarantor(s) listed in this application. I understand and agree to the following:

Scope of Personal Credit Check:
Primeak Inc. may access the personal credit report and credit score of the guarantor(s) to assess the guarantor’s creditworthiness and determine eligibility for the requested loan.

Guarantor’s Consent:
I confirm that the guarantor(s) listed in this application have provided their consent for a personal credit inquiry and understand that this inquiry may affect their credit score.

Verification of Personal Information:
I authorize Primeak Inc. to verify the personal information provided for the guarantor(s), including name, Social Security Number (SSN), date of birth, and address.

Ongoing Credit Monitoring:
I understand that periodic credit inquiries may be conducted on the guarantor(s) throughout the life of the loan to assess compliance with loan terms or determine eligibility for additional credit products.

Adverse Action Notification:
If the loan application is denied based on information obtained from a guarantor’s personal credit report, Primeak Inc. will provide a written explanation to the primary applicant or guarantor, as required by the Fair Credit Reporting Act (FCRA).
